Gonadorelin is synthetic gonadotropin-releasing hormone (GnRH), the native hypothalamic decapeptide. Released in pulses, GnRH stimulates the pituitary to secrete luteinising hormone and follicle-stimulating hormone, which control the hormonal and reproductive function of the gonads.

The GnRH neuronal network generates the pulse and surge modes of gonadotropin secretion critical for puberty and fertility. Kisspeptin neurons in the arcuate nucleus are key components of the pulse generator in mammals, while kisspeptin neurons of the preoptic area are involved in surge generation in female rodents.

In rhesus monkeys whose hypothalamic lesions had abolished gonadotropin release, a constant infusion of the synthetic decapeptide failed to restore sustained secretion, while one pulse an hour — the physiological frequency — re-established it; switching to continuous delivery could then desensitise the release mechanism. Continuous GnRH and long-acting agonists are used on this basis to suppress rather than stimulate the axis.

Tissue and membrane peptidases break the native sequence down readily; superactive analogues substitute a D-amino acid at position 6, which raises receptor affinity and proteolytic stability.

The response depends on delivery: pulses stimulate gonadotropin secretion while continuous exposure suppresses it, so a result with this peptide holds only for the dosing pattern that produced it. The human data come mainly from studies in hypogonadotropic conditions.
