Thymosin Alpha 1 is a synthetic peptide that has been studied for its role in modulating immune pathways and cellular processes. Research has explored its potential applications in areas such as immune response cellular signaling and mechanisms related to homeostasis and tissue dynamics. Its properties have made it a focus of interest in studies involving regulatory functions within biological systems.

T-cell compartment and kinetics

A synthetic polypeptide described as modulating the immune system by augmenting T-cell function, with the proposed route being an action on thymocytes that either stimulates their differentiation or converts them to active T cells. The pharmacokinetics reported in the same review are short: rapid absorption with peak serum concentrations within two hours, a serum half-life of roughly two hours, and a return to baseline within 24 hours.

Hepatitis B record

This is where the clinical literature is densest. A 2001 review summarised 195 patients across four trials, including one study reporting hepatitis B virus DNA clearance at six months in 9 of 17 patients given the peptide against 10 of 16 given interferon alfa-2b and 4 of 15 historical controls, and an open-label trial reporting clearance in 53 percent at six months. A 2015 review of the subsequent work describes monotherapy as suppressing viral replication relative to untreated controls or conventional interferon, with combination against lamivudine or interferon alfa reported to show better DNA suppression and HBeAg seroconversion.

Critical-illness trials

A review of sepsis studies reports reduced mortality, improved HLA-DR expression on monocytes and fewer secondary infections with the peptide alone or in combination, while stating that sepsis is heterogeneous enough that these results cannot be generalised to all patients. An open-label randomised pilot in 49 hospitalised patients with COVID-19 found higher incidence of clinical recovery in the treated group on both low-flow and high-flow oxygen, but neither difference reached significance, with the clearer signal being a 3.84-fold greater rise in CD4+ T cells by day 5 in the low-flow stratum.

The clinical trials behind this compound are in chronic hepatitis B, sepsis and COVID-19, which are not the settings it is generally marketed for. Where the trials are recent they are small and cautious about themselves: the COVID-19 pilot ran to 49 patients and its recovery differences did not reach significance in either oxygen stratum, and the sepsis review states plainly that its results cannot be generalised across so heterogeneous a syndrome. The compound holds no FDA approval.
