Three characterised lines of work

PT-141 also known as Bremelanotide is a synthetic derivative of alpha-melanocyte-stimulating hormone. It has been studied for its interactions with melanocortin receptors particularly melanocortin-4 and melanocortin-1. Research has explored its potential role in influencing physiological processes such as sexual function and immune response as well as its broader effects within these pathways.

Melanocortin receptor agonism

A synthetic analogue of the neuropeptide hormone alpha melanocyte-stimulating hormone, described as having high affinity for the melanocortin type 4 receptor, which is thought to be important for sexual function and gives the compound the potential to modulate brain pathways involved in sexual response. A 2023 review of the central melanocortin system places it alongside setmelanotide and afamelanotide as melanocortin-targeting peptides that reached approval.

Development record

The compound was first described in development as a nasal spray candidate for erectile dysfunction and female sexual dysfunction, with a phase IIb trial in erectile dysfunction completed in 2003 and phase III planned for 2005. It was approved in 2019 in a different form: a self-administered, on-demand subcutaneous therapy for premenopausal women with acquired, generalised hypoactive sexual desire disorder.

Reported adverse events

A safety review covering the whole development programme, 3500 subjects across 43 completed studies with phase 3 exposure of up to 18 months, gives the most common adverse events against placebo as nausea at 40.0 against 1.3 percent, flushing at 20.3 against 1.3 percent, headache at 11.3 against 1.9 percent and injection site reactions at 5.4 against 0.5 percent. Separately, a drug-induced liver injury monograph records mild serum enzyme elevations during therapy and rare instances of clinically apparent acute liver injury.

Bremelanotide holds an FDA approval, but a narrow one, and the rates in the third panel come from that approved dose in that trial population rather than from any other use: nausea was reported in 40 percent of treated participants against 1.3 percent on placebo. Reviews of the condition it was approved for describe its underlying physiology as still unknown. A lactation monograph records that no information is available on clinical use during breastfeeding, and advises caution on that basis rather than on evidence of harm.
