Nicotinamide adenine dinucleotide (NAD+) is a coenzyme found in all living cells. With its reduced form NADH it carries electrons in the redox reactions of energy metabolism, and it is consumed as a substrate by signalling enzymes including the sirtuins, poly(ADP-ribose) polymerases and CD38.

Besides its redox role in glycolysis, oxidative phosphorylation, fatty acid oxidation and the TCA cycle, NAD+ is the co-substrate of sirtuin deacylases, of poly(ADP-ribose) polymerases in DNA repair and of CD38-type hydrolases. These non-redox reactions consume the molecule, and in some tissues its half-life is on the order of minutes.

Tissue and cellular NAD+ levels fall gradually with age in several model organisms, rodents and humans among them, and the decline has been linked to age-associated conditions. NAD+ biosynthesis through nicotinamide phosphoribosyltransferase and SIRT1 activity are described as jointly regulating metabolism and circadian rhythm.

Human trials in this field have mostly studied the precursors nicotinamide riboside or nicotinamide mononucleotide rather than NAD+ itself. In a 10-week trial in women with prediabetes the mononucleotide increased muscle insulin sensitivity; in peripheral artery disease, nicotinamide riboside improved six-minute walk distance against placebo.

Reviews stress that much of NAD+ biology remains poorly understood: separate cellular pools, the microbiome's role in precursor supply and non-canonical breakdown all bear on whether raising NAD+ helps, and the risks of supplementation are listed among the open questions. The human evidence concerns the precursors, not NAD+ itself. The compound holds no FDA approval.
