Cagrilintide is a lipidated, long-acting analogue of amylin, the pancreatic hormone released together with insulin from beta cells and associated with satiety. It was developed to overcome amylin's strong tendency to form amyloid fibrils and the short half-life of the earlier analogue pramlintide.

Amylin's high propensity to form amyloid fibrils makes it a difficult starting point for drug design. Structure–activity work produced a stable, lipidated analogue with a long duration of action, selected for clinical development with obesity as the indication.

The targets examined in this literature are the amylin 1 and amylin 3 receptors and the calcitonin receptor. In work characterising a newer, selective analogue, cagrilintide is described as a non-selective amylin receptor agonist, and it produced more conditioned taste avoidance in lean rats than the selective compound.

Reviews describe amylin's satiating effect as acting on both homeostatic and hedonic regions of the brain, separately from the hypothalamic route of GLP-1 receptor agonists; the two mechanisms are reported to be additive, which is the rationale for studying the analogue together with semaglutide.

Cagrilintide holds no FDA approval. It has been tested alone in a phase 2 dose-finding trial and, mainly, together with semaglutide, with phase 3 results reported in 2025. The most frequent adverse events in its trials were gastrointestinal, primarily nausea, and a review notes nausea as more frequent than with pramlintide.
