Semaglutide is an analogue of human glucagon-like peptide-1 (GLP-1) with two amino-acid substitutions and a fatty-acid side chain that binds serum albumin. It was selected as the long-acting candidate from a series designed to raise albumin affinity and secure full stability against metabolic degradation.

Compared with human GLP-1 the sequence carries Aib at position 8 and Arg at position 34 and is derivatised at lysine 26; the fatty acid and the linking chemistry were identified as the key features for high albumin affinity together with GLP-1 receptor potency. Receptor affinity was three-fold lower than liraglutide's while albumin affinity was higher, and in mini-pigs the plasma half-life after intravenous dosing was 46 hours.

Cryo-electron microscopy structures of semaglutide bound to the GLP-1 receptor–Gs protein complex show peptide interactions similar to those of native GLP-1 but different motions within the receptor and the bound peptide, which the authors relate to the broad spectrum of signalling profiles among GLP-1 receptor agonists.

Beyond its glucose-lowering and weight programmes, the peptide has been tested in outcome trials: cardiovascular events in type 2 diabetes and in overweight or obesity without diabetes, and kidney disease events in type 2 diabetes with chronic kidney disease. An off-treatment extension of one obesity trial followed participants for a year after the treatment was withdrawn and reported regain of weight.

Semaglutide is the active ingredient of medicines approved for type 2 diabetes and, since 2021, for obesity. Those approvals apply to the licensed pharmaceutical products, not to research material such as this vial. Reviews describe the adverse events of GLP-1 receptor agonists as primarily gastrointestinal, and the withdrawal extension above is one reason they treat long-term maintenance as an open question.
