Tirzepatide (LY3298176), here in 30 mg vials, is a single synthetic peptide, modified with a fatty acid, that acts as an agonist at both the glucose-dependent insulinotropic polypeptide (GIP) and the glucagon-like peptide-1 (GLP-1) receptors. It was designed to be long-acting, to test whether GIP-receptor activity adds to the metabolic actions of selective GLP-1 receptor agonists.

In cell lines expressing the incretin receptors the peptide activated both GIP and GLP-1 receptor signalling, and in mice it increased glucose-dependent insulin secretion and improved glucose tolerance by acting on both receptors. Given chronically to mice it decreased body weight and food intake more than a selective GLP-1 receptor agonist.

Receptor occupancy calculated for clinically studied doses shows greater engagement of the GIP receptor than of the GLP-1 receptor. At the GIP receptor the peptide mimics native GIP; at the GLP-1 receptor it favours cAMP generation over β-arrestin recruitment and drives less receptor internalisation than GLP-1. In primary islets β-arrestin 1 limited the insulin response to GLP-1 but not to GIP or tirzepatide.

Like the GLP-1 receptor agonists, the peptide slows gastric emptying; the delay is greatest at first and lessens with continued exposure. Reviews discuss the consequences for the absorption of oral drugs taken alongside, oral contraceptives among them, and for retained gastric contents at endoscopy or anaesthesia.

Tirzepatide is the active ingredient of medicines approved in the United States, the European Union and Japan for type 2 diabetes and for chronic weight management.
